The undersigned is a candidate for employment has indicated that a criminal records check was completed through the employment process at your school, and requests that you release the information to the above listed school.

Pursuant to MCL Section 380.1230 of the Revised School Code, I hereby authorize the listed school and its employees and agents to forward copies of my criminal records check information from the Michigan State Police and the Federal Bureau of Investigation for the purpose of evaluating my qualifications as a candidate for employment. I waive any written notice required under Section 6 of the Bullard-Plawecki employee right to know act, Act No 397 of the Public Acts of 1978, being 235.506 of the Michigan Compiled Laws. I do hereby release the school, its individual board members, employees and agents, past and present, from any and all claims and/or liability whatsoever for any damages or consequences which may result from the pre-employment investigation, including the criminal records check, related to my consideration for employment with the school district and/or as a substitute teacher.
